Investigation Report

The investigation report describes in narrative form how the alleged misconduct was discovered, to whom it was reported, the steps taken to gather the evidence, the applicable standards as derived from documented rules, regulations and administrative issuances, the findings of the investigation, how these findings were evaluated, and finally recommendations made in light of those findings.
